Job Description Job Description ESSENTIAL FUNCTIONS • Provide
goal-directed, strength-based, and recovery-oriented services in
community settings (home, school, etc.). • Collaborate with
clients, families, and treatment teams to develop and monitor
individualized treatment and crisis plans. • Deliver supportive
counseling, emotional and behavioral interventions, skill-building,
and crisis response. • Coordinate services with schools, healthcare
providers, social services, and other community resources. • Assist
with benefit applications, transportation arrangements, and
accessing essential services. • Conduct strengths and needs
assessments, complete and update treatment plans and clinical
documentation. • Serve as an advocate for clients and families and
act as a liaison across systems of care. • Participate in internal
meetings and external networking to enhance service access. •
Maintain confidentiality, promote cultural competence, and adhere
to trauma-informed practices. • Actively support and promote the
agency’s mission and strengths-based philosophy. • Complete
required background checks every two years. • Complete required
training. • Work as part of an integrated treatment team
collaborating with the individual served, family members or others
they want involved in their treatment and other treatment team
members (internal and external) to help support the individual in
their treatment and recovery. • Demonstrate cultural and linguistic
competence. • Approach interactions with individuals served and
colleagues with a trauma sensitive lens • Provide services that are
recovery oriented, and person and family centered, utilizing a
strengths-based approach. • Provide services to meet the whole
health needs of individuals served. • Utilize appropriate
assessment instruments to help monitor the progress of individuals
served and ensure necessary services are received in the
appropriate intensity and duration. • Utilize Evidence Based
Practices (EBPs) as appropriate. • Other duties as assigned by
respective team managers or supervisors. POSITION QUALIFICATIONS
Education/Licensure: • BA/BS degree in Human Services (preferred)
or equivalent combination of education and/or a minimum of 4 years
of relevant work experience in a related field may be considered. •
Highschool Diploma required. Experience: Prior experience working
with children, adolescents, and families preferred. Requirements: •
Must successfully pass the KBI, Drug screen, and all DCF Central
Registry background investigations. • Valid driver’s license,
current auto insurance, a good driving record and a reliable
vehicle are required. Knowledge/Skills/Abilities: • Strong verbal
and written communication • Time management and organizational
skills • Problem-solving and critical thinking • Ability to work
independently and as part of a team • Cultural humility and ability
to engage with diverse populations • Flexibility, reliability, and
adaptability • Maintains professional boundaries and
confidentiality • Proficiency in Microsoft Office and Electronic

economically disadvantaged communities • Occasional physical
activity, including standing, walking, bending, or intervention in

Case Managers must have sufficient ambulatory capabilities to work
with children and adolescents in the community-based setting.
Position may require the ability to use physical holding (MAB) with
children and adolescents exhibiting out of control behaviors.
SUPERVISORY RESPONSIBILITIES This position has no direct
supervisory responsibilities but does serve as a coach and mentor
for other positions in the department. WORK ENVIRONMENT Office and
Community-Based working environment; requires flexible schedule;
frequent interruptions occur; expected to deal with individuals in
crisis; risk of exposure to aggressive and/or acting out (verbal
and physical) children and youth, and exposure to low socioeconomic
homes and neighborhoods.
