Job Description HonorVet Technologies is seeking a travel Home Health Physical Therapist for a travel job in Saint Joseph, Missouri. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: 03/30/2026 - Duration: 12 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist – Home Health Location: Saint Joseph, MO Duration: 13 Weeks Shift: Day Shift – 5x8-Hour (8:00 AM – 4:30 PM); Call required 1 Sunday every 5 weeks; Every 5th week work Saturday & Sunday for admits (2 weekdays off that week) Job Responsibilities: /t - /t Provide Home Health Physical Therapy services to adult and geriatric patients /t /t - /t Perform Start of Care (SOC), admissions, evaluations, reassessments, and discharges (PTs complete their own SOC) /t /t - /t Manage approximately 6 patient visits per day (adjusted when travel distance is high) /t /t - /t Provide care across a 13-county coverage area; rotation to furthest counties weekly /t /t - /t Develop and implement individualized treatment plans and home exercise programs /t /t - /t Complete OASIS documentation and maintain accurate records in Epic (required) /t /t - /t Perform assessments including strength testing, pain assessment, sensory response, vital sign monitoring, skin integrity, environmental barriers, body mechanics, ROM (AROM/AAROM/PROM), adaptive/assistive device assessment /t /t - /t Establish short- and long-term goals, plan of care, RUG levels, and discharge planning /t /t - /t Provide patient and family education including safety, injury prevention, medications, and abuse recognition /t /t - /t Collaborate with interdisciplinary team including PTs, social services, interpretation services, and unit secretary /t /t - /t PTA staff perform routine visits; PT oversees plan of care /t Required Qualifications: /t - /t DPT (Doctor of Physical Therapy) /t /t - /t BLS Certification /t /t - /t Active State License (pending license accepted at submission; must be in hand prior to start) /t /t - /t Minimum 1 year PT experience /t /t - /t Must have Home Health experience /t /t - /t Epic charting experience (required) /t /t - /t Travel experience preferred /t /t - /t Ability to participate in weekend and call rotation /t /t - /t RTO required at submission (1 request allowed) /t /t - /t Must meet 50-mile local radius rule /t /t - /t Modules required (non-billable, completed during orientation) /t Required Clinical Experience & Skills: /t - /t Orthopedics (total knee replacement, hip fractures/injuries, low back pain, osteoarthritis, DDD, DJD, sciatica) /t /t - /t Gait training and extremity mobilization /t /t - /t Post-stroke rehabilitation /t /t - /t Dementia/Alzheimer's and cognitive disorders /t /t - /t Use of assistive devices (rolling walker, standard walker, crutches, gait belt, sliding boards, sit-to-stand lift, hospital beds, lower extremity prosthetics, supplemental oxygen) /t /t - /t OASIS documentation /t /t - /t Home Health documentation standards /t Preferred Skills & Experience: /t - /t Geriatrics, Neurology, Oncology, Sports Medicine, Wound Care /t /t - /t Cardiopulmonary conditions: COPD, CHF, CABG, Emphysema, PVD /t /t - /t Neurologic conditions: Parkinson's disease, spinal cord injury, traumatic brain injury, multiple sclerosis, muscular dystrophy, nerve palsy, concussion, NDT /t /t - /t Orthopedic specialties: pelvic floor training, post-op cervical/shoulder/spinal surgery, prosthetic training, spinal mobilization, taping /t /t - /t Equipment: Hoyer lift, wheelchair assessment, specialty beds, shoulder slings, soft/hard collars /t /t - /t Modalities: TENS, E-stim, ultrasound, moist heat /t /t - /t Understanding of payor requirements /t HonorVet Technologies Job ID 31857531.
